PRACTICE INFORMATION

Holt Medical Centre is a family friendly, long established General Practice operated and owned by locally trained doctors. Our practice specialises in all aspects of family health care such as immunisation and baby health checks; Child and adolescent health; Women’s health including cervical screening, GP shared pregnancy care, Implanon insertion/removal; Men’s Health; Veteran’s Health and DVA claims; mental health care; skin checks and skin lesion removal; minor surgical procedures; travel health and vaccinations; health assessments and care plans, Asthma care and spirometry; and Authorised Commercial and Recreational Diving medicals.

Appointments and Opening Hours

Appointments are available between Monday to Friday 9am to 5 pm. Telehealth appointments are available for existing patients of the practice who have been seen by a doctor in person within the last 12 months..

Our Practice is closed on Saturday, Sunday and Public Holidays.

Appointments may be made by calling the practice during office hours, Monday to Friday. Walk-in appointments are not available.

Please state the degree of urgency. If your problem is urgent, we will endeavour to either see you on the same day, or make other arrangements as necessary. Please also state if you need longer time​ for complicated problems or counselling. In general, appointments will be required for ​all repeat prescriptions, specialist and allied health ​referrals​, medical​ and investigation ​results​. This will ensure confidentiality and enable us to provide appropriate and relevant advice to meet your ongoing health needs. If you, or someone you are assisting, needs an interpreter or has special communication needs, please inform our staff prior to the appointment. 

Cancellations with less than ​24 hour notice, and non or late attendance​ may incur a non-refundable cancellation fee. Please notify reception staff as soon as you know if you are unable to attend an appointment.

 

 

In an ​EMERGENCY​ please dial ​000​, ask for “​Ambulance​”, and state your exact address and nature of the emergency – this is especially important for: any​ ​chest pain, difficulty breathing, severe pain or bleeding, a very sick child, or if you  have been involved in a serious accident.

If you require urgent after hours​ assistance, please contact ​Canberra After hours Locum Medical Service (CALMS) on 1300 422 567 or attend Calvary Hospital Emergency Department or The Canberra Hospital Emergency Department.

Parking and Disability Access

Parking​ is available adjacent to the practice at the Holt shops car park, including​ access​ for people with physical​ ​disabilities​. Disabled toilet facilities are available.

Test Results

If you have been referred for a test, results will usually be discussed at an appointment with your Doctor. Results will not be provided over the phone and reception staff cannot access test results. 

Recall​s and Reminders

Recalls and reminders are incorporated into our medical record system and you may at times receive requests to attend for regular health checks or urgent investigation results. Please discuss with the doctor or practice nurse if you have any queries or do not want any reminders.

 

 

Contacting us

If you wish to urgently discuss a matter with the doctor and are unable to attend the practice, please call Reception staff stating the degree of urgency and discuss arrangements for us to get back to you.

Confidentiality and Medical Records ​

Our practice is committed to maintaining the confidentiality of your health information. Medical records are held securely and if you wish your information to be transferred to another Doctor we will ask you to sign a release declaration. A copy of your relevant information will be sent to the new doctor (with the original file kept at Holt Medical Centre). Medical reports for solicitors and insurance companies will only be performed if appropriate consent has been arranged and prior payment is received. If you wish to have  a copy of your information, it is recommended (and may be required by your Doctor) to book an appointment.

Our practice connects with the My Health Record system. Please advise your Doctor if you do not wish to have your medical records uploaded to this system.

Patient Rights

Patients have the right, and are encouraged, to participate in decisions about their health care.

Feedback and Complaints

Our practice is always willing to listen to suggestions or comments that you may have to improve our patient care. Please speak to a member of the clinical team, or the administrative staff, if you have suggestions or are unhappy with the service you have received. If you have a complaint​ that you feel has not been sufficiently addressed through this mechanism, please ask for a practice complaints form. Alternatively, you may write to the ACT Health Services Commissioner, part of the ACT Human Rights Commission.
